You can complete the fillable PDF below and submit either by:

  1. Print the application and return it to the Board of County Commissioners, Attention Small Business Relief Initiative, via FAX at (513) 732-7366
  2. Email at SBRI@clermontcountyohio.gov
  3. Or mail to 101 East Main Street, Batavia, OH 45103.

Please note that when mailing the application the date and time stamp will be when the application is opened, not when it is mailed.
